Using Your Benefits
Are you currently a member and need help using your benefits?
When Visiting a Physician or Provider:
Locate a participating PPO provider convenient to you. Review your Member ID card and contact your provider network for any questions regarding participating Physicians and facilities. You can also locate an in-network provider by visiting their website.
Schedule an appointment and present your ID card when you arrive.
Your provider will call to verify coverage and determine your out-of-pocket expense.
Please note that some providers may require you to pay any remaining balance after the applicable benefit and discount have been applied at the time of service.
